  3. Scroll to the bottom of the screen and click the “Save” button.

Activity Card Processing Form

This form is for residents who have either lost their Montgomery Village Foundation Activity Card or are new to Montgomery Village and do not have a card yet. Please fill out the required information below in order for staff to quickly process your request. Please do not upload photos with the subject wearing sunglasses, a hat or face mask.

Check this box if you are a current Montgomery Village resident and/or have lost your Activity Card. There is a $5 fee for replacement cards.
